Information for Athletes

Required Physical & Consent Forms

If your daughter intends to participate on any athletic teams at Mercy High School, a yearly physical is required by the NSAA (Nebraska Schools Activities Association). In order to be valid for the school year, this physical must be dated AFTER May 1. This is required of ALL students who wish to participate in any athletics for the school year.

You must complete the parental consent and signatures on the Mercy form, even if you have a different form from your physician.
